Registration

I.  Identification (one of the following is required)

  • Current PA Drivers license/Non-Drivers license
  • Valid Federal State or Municipal employment photo identification

AND

II.  Proof of Residency (one of the following is required)

  • Current Utility Bill (PECO, PGW, Water)
  • Current Voter Registration
  • Original Rental Agreement/Leases (must be typed, signed by all parties & notarized)
  • Deed
  • Residency Affidavit signed by landlord and notarized

Kindergarten Registration MUST ALSO include the following

  • Complete immunizations including two MMRs and two Varicellas
  • Birth certificate of the child.  A baptismal record is accepted.

Permission Forms

School Trip Permission Form – All students going on school trips will receive a permission slip from their teacher to go on the trip.  This form must be signed by a parent or guardian and returned before a student is allowed to attend any trip.

Extracurricular Activities

Prince Hall encourages all students to participate in extra-curricular activities.  Please pay attention to the daily announcements, quarterly newsletter and flyers sent home from school for information regarding extra-curricular activities at Prince Hall.  If a student decides to participate in any activity, the student must:

  • Have permission form signed by a parent or guardian.
  • Arranged to be picked up promptly by an adult or have permission to walk home.
  • Display outstanding character and sportsmanship.
  • Adhere to all rules and regulations.
  • Follow conflict resolutions.
  • Attend regularly
